How To Apply

Step 1: Pre-Application Screening

Applicants first complete a short Pre-Application Form to determine if they may qualify for assistance.

This form asks basic information such as:

• Pet owner name and contact information

• Pet name and type

• City and state

• Brief description of the pet’s medical need

• Estimated veterinary cost

• Name of veterinary clinic

This step helps our team review requests quickly and prioritize urgent cases.

Step 2: Pre-Application Review

Our team reviews the pre-application to confirm the request meets basic eligibility requirements.

Applicants may receive one of the following responses:

• Invitation to complete the full application

• Request for additional information

• Notification that the program cannot assist at this time

Step 3: Full Application Submission

If the pre-application is approved, the applicant will receive a link to complete the full Luna’s Impact Assistance Application.

This form collects more detailed information about the pet, the medical situation, and financial need.

Step 4: Submit Required Documents

Applicants must upload supporting documents with their application:

• Proof of pet ownership

• Proof of Massachusetts residency

• Veterinary estimate or medical records

• Proof of financial hardship

Applications cannot be reviewed until all required documents are submitted.

Step 5: Application Review

Our team evaluates the request based on:

• Medical urgency

• Financial need

• Available funding

• Program eligibility

Step 6: Decision Notification

Applicants will receive an email notifying them if their request is:

• Approved

• Partially approved

• Waitlisted due to limited funding

• Declined

Because funding is limited, assistance cannot be guaranteed.

Step 7: Payment Arrangement

If approved, assistance may be:

• Paid directly to the veterinary clinic, or

• Provided as partial financial support toward the veterinary bill.

Step 8: Follow-Up & Pet Updates

Recipients will be asked to share updates about their pet after treatment so the Luna’s Impact community can see the difference their support makes.
